Penggunaan thiazide diuretik biasanya dikaitkan dengan kehilangan elektrolit, paling jauh kalium tetapi juga sodium klorida, bikarbonat, dan magnesium. Kehilangan lain elektrolit seperti fosfat, bromida dan iodida biasanya sedikit. Kalium, magnesium yang kekurangan boleh membawa kepada aritmia jantung dan serangan jantung. Lain elektrolit berkaitan dengan komplikasi termasuk alkalosis metabolik dan hyponatremia, yang jarang mengancam nyawa. Terapi dengan thiazide diuretik perlu diberikan berhati-hati pesakit atau terdedah kepada cecair dan elektrolit kekurangan, termasuk pasien dengan rendah atau menengah aldosteronism (mungkin telah rendah potassium); orang-orang dengan teruk atau berpanjangan diare atau muntah-muntah dan orang-orang miskin dengan nutrisi status.
