Thiazide diuretik mungkin tidak berkesan apabila glomerular kadar penapisan adalah rendah (GFR < 25 mL/min) karena mereka tidak dijangka akan ditapis ke renal tubul, mereka tapak tindakan. Di samping itu, thiazide diuretik penurunan GFR dan mungkin mengendapkan azotemia dalam penyakit ginjal. Terkumpul kesan mungkin juga membangunkan karena kebanyakan ubat ini dikeluarkan tidak berubah dalam air kencing oleh glomerular penapisan dan aktif tiub rembesan. Terapi dengan thiazide diuretik perlu diberikan berhati-hati di dikurangkan dos dalam pasien dengan buah pinggang. Jika fungsi ginjal menjadi semakin buruk, seperti yang ditunjukkan oleh kenaikan BUN atau serum peluang untuk bertemu muka peringkat, gangguan atau pemberhentian thiazide terapi perlu dipertimbangkan.
