Phenothiazines banyak kekal oleh hati dan mungkin berkumpul di dalam pasien dengan hati kemerosotan. Di samping itu, menggunakan beberapa phenothiazines telah dikaitkan dengan buruk hati kesan termasuk penyakit kuning kolestatik dan enzim hati tinggi, umumnya dalam beberapa bulan pertama terapi. Penyakit kuning kolestatik biasanya terjadi antara kedua dan keempat minggu terapi dalam kira-kira 0.1% 4% dari semua pasien. Terapi dengan phenothiazines perlu diberikan berhati-hati pesakit dengan wujud terlebih dahulu penyakit hati, hati enzim yang tidak normal, atau hepatitis. Fungsi hati dan air kencing bilirubin ujian yang harus dilakukan secara berkala semasa berpanjangan terapi, dan pesakit harus diarahkan untuk segera laporkan apa-apa tanda-tanda atau tanda-tanda tidak senonoh kolestatik penyakit kuning seperti sakit perut atas, mual, kulit kuning, seperti influenza gejala, ruam, dan demam. Phenothiazine terapi harus dihentikan, sebaik-baiknya selama-lamanya, jika penyakit kuning berlaku dan boleh dikaitkan dengan dadah. Klinis pemulihan biasanya diperhatikan dalam beberapa minggu berikut pengeluaran terapi, walaupun histopathologic perubahan mungkin bertahan lebih lama.
