Tricyclic dan tetracyclic antidepresan (TCAs) telah antikolinergik aktiviti, untuk yang pesakit tua adalah sangat sensitif. Tinggi amina seperti amitriptyline dan trimipramine cenderung untuk pameran lebih besar antikolinergik kesan daripada agen yang lain di kelas. Terapi dengan TCAs perlu diberikan berhati-hati pesakit sudah ada keadaan yang mungkin akan lebih buruk dengan antikolinergik aktiviti, seperti kencing penahanan atau halangan; sudut penutupan glaukoma, tidak dirawat kalau tekanan darah tinggi, atau tidak terkawal rendah sudut terbuka glaukoma; dan pencernaan penghalang gangguan. Pada pasien dengan sudut penutupan glaukoma, walaupun rata-rata dos boleh mengendapkan serangan. Glaukoma harus dirawat dan di bawah kawalan sebelum permulaan terapi dengan TCAs, dan kalau tekanan dipantau selama terapi.
