Phenothiazines boleh menyebabkan akut, dos yang berkaitan dystonic reaksi menengah untuk pusat dopaminergic blokade. Ini adalah reaksi mempunyai ciri-ciri spastik kontraksi diskrit kumpulan otot dan mungkin termasuk torticollis, opisthotonos, carpopedal kekejangan, trismus, kesukaran menelan, perioral kekejangan dengan bertopi lidah, dan krisis oculogyric. Bermulanya biasanya dalam 24 96 jam berikut permulaan terapi atau meningkatkan dos. Faktor risiko termasuk usia muda, kelamin laki-laki, gunakan tinggi potensi agen (contohnya, fluphenazine, perphenazine, trifluoperazine), dos yang tinggi, dan IM pentadbiran. Terapi dengan phenothiazines perlu diberikan berhati-hati pesakit, terutama anak-anak, dengan hypocalcemia atau teruk dehidrasi, sejak pesakit ini mungkin lebih mudah untuk dystonic reaksi.
