Phenothiazines mungkin jarang menyebabkan hematologic keracunan, termasuk agranulocytosis, thrombocytopenia, eosinofilia, aplastik anemia, purpura, granulocytopenia, dan hemolitik. Ringan leukopenia boleh berlaku kerap dengan dosis besar lebih tempoh yang lama tapi biasanya balik walaupun terus rawatan. Terapi dengan phenothiazines perlu diberikan berhati-hati, jika sama sekali, pada pasien dengan wujud terlebih dahulu darah dyscrasias atau tulang sum-sum penindasan. Darah lengkap tuduhan harus didapati secara teratur, dan pesakit harus diarahkan untuk segera laporkan apa-apa tanda-tanda atau tanda-tanda tidak senonoh darah dyscrasia seperti demam, sakit tenggorokan, tempatan jangkitan, pendarahan, pucat, pening, atau penyakit kuning. Kebanyakan kes agranulocytosis telah berlaku antara keempat dan kesepuluh minggu terapi.
