Tricyclic dan tetracyclic antidepresan (TCAs) diketahui untuk menjalani metabolisme di dalam hati. Beberapa metabolik, seperti orang-orang imipramine, clomipramine dan desipramine, mungkin farmakologi aktif. Banyak metabolik juga dikeluarkan oleh buah pinggang. Ada yang sangat terhad data mengenai penggunaan TCAs pada pasien dengan buah pinggang dan/atau penyakit hati. Terapi dengan TCAs perlu diberikan berhati-hati pesakit jauh penglihatan buah pinggang atau fungsi hati. Dos pelarasan yang mungkin perlu.
