Tricyclic antidepresan (TCAs) boleh memburukkan lagi gejala psikosis di pesakit skizofrenia, terutama orang-orang dengan paranoid simptomatologi. Pesakit tertekan, biasanya orang-orang dengan gangguan bipolar, mungkin mengalami beralih dari kemurungan untuk mania atau hypomania. Kejadian ini juga telah dilaporkan jarang dengan tetracyclic antidepresan, maprotiline. Terapi ini dengan agen perlu diberikan berhati-hati pesakit dengan skizofrenia, gangguan bipolar, atau sejarah mania.
